Lifebar Character Displayname and title

I have seen many lifebars that have a character and its (example) displayname = "Alex". My question is how do I add a "title" to this like say I call him "The Manhattan Commando". Do I add this to the .def or is it something else?

You edit the character's .sff file. Their large portrait.

The title is actually part of the lifebar image.
